Summary
Clinical data shows that the St Oswald’s ward of Netherton has significantly lower dementia diagnosis rates than both the rest of Sefton Borough and nationally.
In response, Sefton CVS has led a collaborative project with ReCITE, funded by Liverpool School for Tropical Medicine, to address these low diagnosis rates through innovative and creative, community-led approaches.
The Community Innovation Team (CIT) brings together partners from across the system, including Living Well Sefton (CVS), South Sefton PCN, Sefton Public Health, Alzheimer’s Society, Healthwatch, L30 Centre, Glovers Lane GP Surgery and Sefton Memory Clinic, alongside a wide range of local voluntary, community and faith (VCF) organisations. Through this partnership, professionals have shared perspectives and data, building a richer collective understanding and strengthening relationships across the system.
At the heart of this work is genuine co-production with local residents, combining clinical data with community insight to ensure that lived experience shapes both the understanding of barriers and the development of solutions.
This presentation will explore how a creative, community-driven approach has helped to tackle dementia awareness and diagnosis in Netherton, and how the learning, partnerships and co-produced methods offer a scalable and replicable model for other communities seeking to address health inequalities.
