Summary
This presentation explores how Admiral Nurse clinics across the UK have become more than places of clinical support evolving into hubs for connection, collaboration and community engagement. Delivered in accessible, non-clinical settings, these clinics embed specialist dementia support within local communities while strengthening relationships with families, community groups and partner organisations.
The session highlights how co-production, partnership working and local intelligence are used to understand community need, improve access to support and develop responsive pathways for people affected by dementia. Through practical examples, including walking football, barn dances and community outreach initiatives, attendees will see how innovative approaches can increase visibility, reduce barriers to support and engage underserved communities.
The presentation also explores how working in community settings promotes adaptability, shared learning and collaborative leadership across teams and hosted services. Delegates will gain transferable insights into community engagement, partnership working and service innovation, demonstrating how meaningful connections can improve outcomes and build resilience for families living with dementia.
