Summary

The presentation and poster will outline the Dementia UK Frailty Consultant Admiral Nurse service offer, describe the current frailty landscape alongside reporting on both Quantitative and Qualitative outcomes pertaining to operational practice for the first 2 years of service.

Lived Experience partners were consulted to ensure that co-production was evident at each stage of service planning, and delivery.

The service is evaluated against Qualitative measures looking at value and experience-based measures. The service focuses on the needs of families living with dementia and frailty; and aims to reduce barriers to care and support and equality in service provision for those families requiring support.

The service is delivered by Consultant Admiral Nurses.
Admiral Nurses are specialist dementia nurses who provide psychological support, expert advice, and information to help families affected by dementia, particularly those with complex needs.
