Summary
Patients living with dementia arriving at the operating department for surgery, did not have information pertinent to their care needs. A clinical audit was conducted to assess the current state of persons living with dementia arriving for surgery with supporting documentation at the trust and compliance with policy.
Pre-intervention data showed that 0 out of 12 patients captured over a 3-month period had the "This is Me" document. The "Living with Dementia theatre passport” was co-produced with stakeholders, surgical departments and lived experience partners. It was introduced to enhance patient experience, improve communication and present relevant information, clearly and concisely.
Post-intervention, after 12 months 15 patients were captured over a 3-month period. 12 had "Living with Dementia theatre passports” and 3 had "This is Me" documents. Qualitative feedback from patients, carers and healthcare staff was overwhelmingly positive. Endorsements have been received from the Lewy body society, Johns Campaign, house of memories and Dementia United.
