Summary
Since November 2021, the Mapping, Education and Carers Service (MECS) at Aneurin Bevan University Health Board (ABUHB) has embedded Dementia Care Mapping (DCM) to enhance person-centred care for people living with dementia. Delivered by a specialist multi-disciplinary team, the service provides DCM observations, staff education, and support for carers. The team has collaborated with Complex Care teams and Care Inspectorate Wales to improve care in homes under special measures and address placement breakdown concerns. DCM has also led to the development of several practice change models. These include models to promote respectful, person-centred communication and reduce undignified language in care settings. Additionally, DCM supports patient wellbeing through meaningful engagement in outdoor spaces. DCM is recognised in the All Wales Dementia Care Pathway, with plans for further expansion into general hospitals, day services, and prisons.
