Sophie has worked within health and social care for over 16 years. She is a Specialist Admiral Nurse for Young Onset Dementia. Sophie has experience of working within a Specialist Dementia Palliative Care service and Leading a Primary care Admiral Nurse service. Working in both the acute and community settings. Currently working in a national role at Dementia UK. Sophie is passionate about the inequalities of dementia care and wishes to advocate for increased specialist support for all of those affected by Dementia.
Advance care planning (ACP) is the process of discussing and recoding future wishes and preferences. ACP is an important process for those living with Young Onset Dementia. As dementia progresses, individuals may find it hard to communicate their wishes and make complex decisions. There are many barriers that prevent people with Young Onset Dementia from exploring ACP. The workshop will be designed to explore advance care planning conversations and templates available to record/ share an advance care plan for those living with Young Onset Dementia.
