Summary

Advance care planning (ACP) is the process of discussing and recoding future wishes and preferences. ACP is an important process for those living with Young Onset Dementia. As dementia progresses, individuals may find it hard to communicate their wishes and make complex decisions. There are many barriers that prevent people with Young Onset Dementia from exploring ACP. The workshop will be designed to explore advance care planning conversations and templates available to record/ share an advance care plan for those living with Young Onset Dementia.
