Jan is a registered nurse and worked for many years at Imperial College Healthcare NHS Trust in the Complex Discharge Team. She has been the Operational Led for the MinderCare Clinical Service since it's inception in April 2024.
This presentation will share our experiences of setting up and running the MinderCare service. This is a nurse-led blended clinical and research program in West London which is exploring the sustainability of digitally enabled support for clinical decision making. Part of the project has been to explore the feasibility of delivering person-centred nursing care and support to people living with dementia and their families or carers in their own homes. This presentation will share the experiences of the nursing team and talk about the challenges and rewards that we have encountered so far.
