Dementia 100 Pathway Self-assessment Tool launched
The Dementia 100 Pathway Self-assessment Tool has been launched.
The Tool has been developed through collaboration between NHS Midlands and Lancashire’s Improvement Unit, the Department of Health and Social Care (DHSC) and NHS England (NHSE). It’s been supported by the National Collaborating Centre for Mental Health, the Alzheimer’s Society and Dementia UK.
Dementia 100 is described as providing Places and Integrated Care Systems (ICSs) with a standardised and evidence-based framework to achieving consistent, integrated, high-quality care across all areas of dementia provision. The Tool does this by:
- Assessing quality: Conducting a comprehensive evaluation of dementia care provision to identify strengths and areas for improvement.
- Reducing variation and creating economies of scale: Enabling systems to take more efficient approaches to the delivery of dementia services and work in a more collaborative way across neighbourhoods and Places.
- Driving strategy development: Supporting the creation of evidence-based strategies for service redesign and commissioning, aligned with the RightCare Scenario and key national policies.
- Sharing best practice: Fostering collaboration across systems by highlighting areas of excellence and facilitating the exchange of successful approaches.
- Informing future service development: Guiding the design and implementation of services that improve the support available to individuals living with dementia and their carers.
