Evidence wanted documenting interventions or innovations in frailty and/or dementia care
NHS England and the Department of Health and Social Care (DHSC) are inviting examples of interventions or innovations for frailty and/or dementia care that improve quality and performance of services as part of an evidence gathering exercise for the Modern Service Framework (MSF) for Frailty and Dementia.
Interventions or innovations can be at any stage of maturity, including those that are already delivering impact or programmes which are no longer active, as well as emerging models.
