New resources available to support care homes with person-centred care planning
New resources to support person-centred care planning for older adults in care homes are now available.
Created with input from over 100 health and social care professionals and residents’ family and friends, the resources are intended to make care planning more meaningful, effective, and inclusive. They have been designed to support everyone involved in care planning—from senior staff to care assistants—and to encourage collaboration with residents and their families and friends.
