Young onset dementia care framework published
Dementia UK and the Young Dementia Network has published a young onset dementia care framework.
The framework has been developed to help integrated care boards (ICBs) meet the needs of people with young onset dementia. It is hoped the framework will help
support care systems to deliver:
- Faster, more accurate diagnoses
- Integrated, age-appropriate support at every stage
- Named young onset dementia leads within both the ICB and clinical services
- Better outcomes and more efficient use of resources
The developers of the framework say:
“The framework offers clear, evidence-based steps to deliver best practice care, including a gold standard care pathway providing guidance from diagnosis to end of life. The framework is designed to complement the Dementia 100 toolkit (https://journalofdementiacare.co.uk/dementia-100-pathway-self-assessment-tool-launched) and Well Pathway for Dementia (https://www.england.nhs.uk/mentalhealth/wp-content/uploads/sites/29/2016/03/dementia-well-pathway.pdf), but offering tailored interventions and advice for supporting those diagnosed before 65.”
